Education: BS in Pharmacy or Doctor of Pharmacy (PharmD) from a college of pharmacy accredited by the American Colleges of Pharmacy or its foreign equivalent as determined by credentials evaluation or by an FPGEE Certificate and eligibility for Ohio Pharmacist Licensure.

Summary: The Cleveland Clinic in Cleveland, Ohio seeks an Inpatient Pharmacist III to, under the general direction of the Manager/Coordinator, assure appropriate control, security, prescribing, preparation, dispensing, and utilization of pharmaceuticals; maintain control and security of all drugs; review, interpret, clarify, enter, and verify patient information and medication orders for processing within all hospital and pharmacy computer systems; advise prescribers of medication-related issues and recommend alternate medications; provide general and patient specific drug information to patients, physicians, and nurses and other health care professions; check final processed orders and approve for delivery or release; verify all products prepared and source containers and amount used for accuracy; perform routine pharmacotherapeutic monitoring; interact routinely with physicians, nurses and other health care professionals to provide optimal pharmaceutical care to patients; maintain appropriate documentation of all activities including clinical interventions and follow-up in problem orders; precept pharmacy interns and introductory practice experience; review activities of Pharmacy support personnel; support formulary and restricted drug programs; participate in pharmacy educational development programs.
